Hyperkalemia can be a life-threatening disorder, especially for patients with heart failure (HF). HF patients at particularly high risk include those with the common comorbidities of chronic kidney disease and/or diabetes, and individuals of advanced age. Renin-angiotensin-aldosterone system inhibitors (RAASi), including angiotensinconverting enzyme inhibitors (ACEi), angiotensin receptor blockers (ARB), and mineralocorticoid receptor antagonists (MRA) decrease morbidity and mortality in HF but increase the risk of hyperkalemia. Unfortunately, due to the development of hyperkalemia, these life-saving therapies are discontinued, which may place patients at heightened cardiovascular risk. Sometimes less than optimal dosages of RAASi are prescribed as a preventive strategy. This may be due to clinician fear of the development of hyperkalemia. Treatment of hyperkalemia depends on the severity and cause. Although there are effective therapeutic options for acute management, traditional strategies for chronic control of high potassium levels can have limitations. New potassium binders have demonstrated effi cacy with a more acceptable safety profi le, and may allow continuation of RAASi. In this program, Chair Dr. Javed Butler and faculty members Dr. Akshay Desai and Dr. Mikhail Kosiborod will explain the physiology and pathophysiology of potassium homeostasis, identify causes of hyperkalemia, suggest prudent preventive strategies, describe goals of treatment of acute vs subacute/intermediate vs chronic hyperkalemia, and discuss effi cacy and safety of traditional and new therapies. State-of-the-art presentations will be followed by a case study with panel discussion in which commonly encountered barriers will be identifi ed and best practices recommended.
This educational activity is intended for cardiologists and other healthcare professionals involved in the management of patients with heart failure.
